Community Care Coordinator roles at Whiddon
As a Community Care Coordinator at Whiddon, you’ll support older Australians to live independently at home by supporting our teams to deliver personalised, high-quality care.
You’ll manage all aspects of client care from referrals and admissions to care planning, reviews, and service delivery. Working with clients, families, providers, and internal teams, you’ll ensure services are compliant, person-centred, and responsive to individual needs.
You’ll also monitor Home Care Package budgets, oversee CHSP services, and support Home Care Employees through coaching, compliance checks, and performance management. As part of the broader team, you’ll contribute to rostering, recruitment, and the on-call rotation making a meaningful difference every day.
