How To Navigate Aged Care Assessments

Navigating aged care assessments are an essential step in ensuring you or your loved one gets the right support to maintain independence and wellbeing – whether that’s support at home, residential care or other services.

Recently, the process underwent significant changes with the introduction of the Single Assessment System, aimed at making assessments more streamlined and consistent for everyone.

In this guide, we’ll explain what an aged care assessment is, how to prepare, and what to expect in light of these updates.

What Is An Aged Care Assessment?

An aged care assessment is designed to help individuals access the care and services they need to maintain their health, independence and quality of life. Previously, assessments were carried out by three different workforces including the commonly known Aged Care Assessment Teams (ACATs) and Regional Assessment Service (RAS).

Now, with the introduction of the Single Assessment System in December 2024, all assessments are conducted through one unified framework, ensuring a consistent, person-centred approach across Australia.

This new system was designed to simplify the process by merging the previous pathways into one, making it easier for individuals and families to navigate. The assessment focuses on:

  • Current health and medical conditions.
  • Ability to manage daily living tasks.
  • Social and emotional wellbeing
  • Personal preferences for care

Key Steps To Prepare For Your Assessment

Here are some practical ways to prepare for an aged care assessment and make the process as smooth as possible:

1. Understand the Changes

Familiarise yourself with the recent updates to the system. The Single Assessment System now means all aged care assessments follow the same process, regardless of whether you’re seeking home care packages or residential aged care.

(*Please note that From 1 July 2025, the new aged care Support at Home program will replace the existing Home Care Packages Program and Short-Term Restorative Care Programme. Find out more information here.)

2. Contact My Aged Care

Now it’s time to register with My Aged Care, the gateway to access government-subsidised aged care services in Australia. They will help guide you through the initial steps, help check your eligibility and connect you with an assessor.

3. Organise Important Information

Gather the relevant documents such as medical records, a list of medications and details of any existing care arrangements. These will help the assessor build a complete picture of your or your loved ones needs.

4. Involve a Trusted Loved One

Whether it’s a family member or close friend, having someone you trust by your side can provide extra reassurance and help communicate needs during the assessment.

5. Prepare Questions for the Assessor

Write down any questions or concerns you might have, such as understanding how home care packages work, what residential aged care options are available, or what to expect after the assessment. This is your opportunity to clarify any uncertainties and ensure you feel confident moving forward.

What To Expect During the Assessment

Here’s what will typically happen during the assessment:

  • Initial Contact: An assessor will contact you to confirm eligibility and schedule a meeting.
  • Face-to-Face or Virtual Meeting: During the assessment, the assessor will discuss your health, daily activities, and the challenges you face.
  • Holistic Approach: The assessor will evaluate your physical, social, and emotional needs to ensure all aspects of your well-being are considered.
  • Explanation of Care Options: Based on the assessment, the assessor will provide information about available services that best suit your needs.

What Comes After The Assessment?

Once the assessment is complete, you’ll receive a detailed summary of the recommended services. Here’s what happens next:

  1. Review the Report: The report will outline the aged care services and supports you’re eligible for, including specific funding options like home care packages.
  2. Choose Your Care Provider: Use the information in your report to research and select a trusted provider, such as Whiddon, to deliver the recommended care services.
  3. Apply for Services: Work with your chosen provider to begin accessing the approved services.
